So this happened yesterday in Lodi CA. Guy in wheelchair stuck on the tracks. Cop tries to pull him to safety. Appears at least one of his legs got amputated, but at least the man is still alive.

I was a conductor one time and we hit a pickup and trailer. The engineer made a brake reduction but didn't big hole it. He said if I big holed it, we wouldn't have slowed down much and I would run the risk of flattening wheels. The crash shows the value of the ADA rubber inserts in the flange-way.
