Mother and child almost fall through floor of moving train

This is a strange story. In England, a mother and her 3-year old child almost fell through the non-existant floor in a toilet while on a moving train. The door to that toilet had been secured with screws, but apparently the screws vibrated loose. This seems to have been some vintage equipment.
